give the permission for my child to attend the Madrasah "Janazah workshop" on Tuesday 29-Mar-2022 from 04:45 AM till 06:45 AM.
I give permission for my child to walk or ride in any vehicle designated by Madrasah Zeenatul Quran, its employees and volunteers, while participating in and travelling to and from this event.
I agree to accept full responsibility, financially or otherwise, for any damage my child may do to the properties visited on outing, others personal property, or vehicles used for transportation.
In case of an emergency, I give permission for my child to receive medical treatment.
I give permission for photographs of my child to be taken and displayed on the Madrasah website, local newspaper, TV channel or social media.
I agree and consent to all of the above stated.
